Parent Authorization Forms

Good afternoon,

In order to participate in the Cappies, students must submit a Parent Authorization Form, which you will find attached to an email that was sent to critics, directors, advisors, and mentors on 9/23. Critics, please print this form and bring a signed copy to trainings this week, which we will collect at the sign-in desk.

Following this week's trainings, we will send a list of students missing Parent Authorization Forms to each Advisor. Advisors should then collect forms from all missing students, and email them to AdminNCA@cappies.org. Please try to send all missing forms in one email for ease of tracking. Late forms are due by October 7.

We strongly encourage anyone who is able to attend an in-person training session.
